[Plugin: Navayan Subscribe] Double heading in form, how to remove?

I am using Navayan Subscribe form.
There is this text “Unsubscribe”, above my dutch translation.
You can see it here : https://www.polaroidz.com , its the left sidebar.
I cant find anywhere in the code to get rid of that. Does somebody know?
