Looking for plugin for sign up form

Hi,
I’m looking for a plugin that will create a form on a page for people who want to sign up or belong to my organization. It will need their name, address, phone number and email address.

I wont’ be collecting dues from them online but asking them to mail in their dues once a year to a specific address.

Any suggestions? It seems pretty simple. Also, can I do this through Mail Chimp?
