[Plugin: WP Customer Reviews] Cannot make it work

I am actually having a problem too, i see the stars and the name of the reviewer but nothing else ??

Can some one help i have just installed the WP Review plug in but when i go to activate i am getting the following

COULD NOT CREATE DATABASE TABLE, PLEASE REPORT THIS ERROR

has any one had this problem
