[Plugin: TDO Mini Forms] Some submissions not coming through (possibly)

I have emails from a couple of people over the last 2 weeks who have submitted a post (or they believe they have) but I have no record of any post from them.

It could be that they have just not submitted the form properly, or to do with downtime on my hosting, but it seems too much of a coincidence that 2 people have contacted me.

Also my submissions have slowed down considerably in the last 1-2 months so I wonder if this has been happening more often. I have since had 2 submissions through, so some are working, but these ones have not.

Has any else experienced this?
