[Plugin: Fast Secure Contact Form] Checkbox not configured correctly

I have two questions relating to membership on my form:
– STC member?
Error: A checkbox field is not configured properly in settings.
– Washington, DC – Metro Baltimore chapter member?

Both are configured the same way but there is red text between the two that says a checkbox isn’t configured properly. The Help says for a checkbox with one option, all I need to do is put in the label. It isn’t a required option so that is not set. We want to know if you are a member of the Society and the chapter or just the Society.

The form won’t send with this error. When I try to send a test with both checkboxes checked, I get “Please make corrections below and try again.”
and the line about improper configuration is repeated below the first line. Also, only the first question retains a check in the checkbox. The second question has been unchecked. How do I fix this? Can we not have a list of things to check or not check? For example:
-I can judge pubs
-I can judge online material
-I can play a piano
-I like dogs
etc.

I looked at the source code of the page and it shows a value=”selected” for both of them. Where did that come from? It isn’t selected when you look at the form and I don’t want it to be selected–it isn’t required. Just check it if it is true. How do I get rid of the value field for a checkbox?
