User-submitted photos and search

Is there a way to customize the fields where users submit their photos? Basically I want to users to be able to upload their surgery results so that visitors can browse photos by by surgeon or by surgery type.

I’ve never created anything like this on any website I’ve run so this is new territory, some guidance as to how I could accomplish this would be awesome!
