Problems with e-mail confirmation

I have a problem with e-mail confirmation. After subscribing on my site I get e-mail. When I click “Confirm” link in e-mail body I get error page with following text:
ERROR # 5 (unknown user) ! Check you are using the appropriate link.
What should I do to resolve this problem? Thank you.
