Data entry form as template for a post

Extension that allows you to create a form to use in a post. For example, you click it while writing a post and it gives you this:

Name: [textbox]
Email: [textbox]
and so on

And when you submit it enters this in your post:

Name: TEXT ENTERED FROM NAME
Email: TEXT ENTERED FROM EMAIL

I bet something like this exists but I don’t know its name (I’ve searched for “data entry”, “post template”, “post form” without luck). Also I’d like to know the most customizable one (you get to choose from many fields, not just textboxes, you can customize the text that will be displayed in the post and you can upload images, of course if there is an extension that does that). Thanks.
