Form that Emails User w/ File Attachment

  • Hello,

    I am looking for a plugin that works as a contact form.

    – User inputs information
    – It is saved to a database
    – and lastly, they receive an email with a file attachment.

    The file attachment detail is key.

    Does anyone know of a plugin that can accomplish this? Thank you.
