Dynamic/Growing List Form

I’m trying to find a plugin or an example of a form that allows the user to add text fields by clicking a button (a plus for example).

The functionality would be like building a list, some people might have 5 things to submit, some might have 20. So rather than giving them a default 20-30 (of which most might be empty and taking up space) they could add as needed.

Has anyone come across something like this?
