Need to add an image field to user meta

I have just purchased Extra User Fields 2, and am having difficulty understanding how to implement it. I need to add an img or file-upload to my user form so that I can associate a company logo with each person. I was successfully able to add TEXT phone {Phone:Please enter your office phone number}. But I don’t see instructions for the type I want. Also, if you could give any hints on how to call that information in my template after it was uploaded, I would appreciate it.
