    Can you provide a link to your site with the plugin active?

    Another user was able to get it work using these settings:
    Form action: https://app.icontact.com/icp/signup.php

    E-mail Field: fields_email

    For the form action, try only https://app.icontact.com/icp/signup.php. Right now, there’s a longer list of attributes attached to it in the setting, which is being converted to other characters to not break the html, but in this case, it is breaking the form.
