works with code in 'other notes' but not anywhere else

I can’t seem to get the plugin to work automatically.
I’ve built a theme using _s, so I’m relatively sure wp_head and wp_footer are in the right place, and the scripts are loading when the plugin is active. Neither position seems to make any difference.
The plugin must be almost working though because I can get it to work with static images I swap out in the header etc. using the code in ‘other notes’.
Have disabled plugins and no luck.
Running this site as a subdomain on a multisite that’s been domain mapped. The way I write the url in the ‘other notes’ code doesn’t make any difference ie. mapped.orig-domain.co.uk/image.png, mapped.com/image.png, orig-domain.co.uk/image.png, all work with the snippet provided.
Not seeing any automatic change with uploaded images though! Does this only work with new images? Do I have to transplant the snippet provided manually on to every location where an img is written? I’m absolutely stumped! Anyone else had this issue?
