I want to use this textbox in a Astrology Website to submit client question. But before submitting any question client need to fill his personal information like email, phone and more.

I already have bbpress forum plugin in my website. I don’t need any forum plugin. When client fill and submit his question then i will get that question by email. I need something like this.

Above footer (Right Hand Side) You will see the tab function. I need same tab function like this. Please help if you know any plugin or know how to made.
