VFB not creating database tables for new sites on multisite

Hi Everybody,
Our company uses multisite to run a very large network of sites, and we’ve been using Visual Form Builder Pro to create our forms very quickly and very successfully for some time now. All of a sudden it seems to have lost the ability to create new forms on new sites.
I click “Add New”, I fill out the basic information, then click “Create Form”, and the page refreshes and reverts back to the empty “All Forms” page, and all of the information I had put in disappears. I can’t create new forms unless I manually import database tables from another site into the the new one, and then it works fine after that…
Does anybody know why this would happen? I’ve searched and searched but have found nothing. I love this plugin and would really like to continue using it!
