Huge Postmeta Table Timing Out Site

I have a worpdress site running a custom theme, with lots of custom functionality. The posts table has about 30,000 rows, and the postmeta table has about 300,000 rows. The user / user_meta tables have about the same.
The problem hasn’t been with the custom installation, and functions, but with the default wordpress admin pages. Anytime I load a regular post type at edit.php?post_type=… it pretty much times out.
I have other custom admin pages which work fine with the same amount of rows.
What does the wordpress admin do that would be so different? is there a cache that is killing it in the middle?
