creating custom event submission form for a category

I would like to create a custom event submission form for a specific category of events.
Can we create different event-forms? Is there a template for the event form that can be edited?
If it requires css styling, what specifically do I need to change?
In the default event form I want to make the following changes.
“EVENT NAME” changed to “Church Name”
NO date included
Location can stay the same.
No category option – all would be the same.
Allow details, custom image, but no bookings.
Is this possible?
