Questionairs with next and previous quesiton

I tried but did not get anyone near to which i want

I need some thing like this

1 2 3 4 5 6

1. Question, how are you
Ans text field [in morning]
Ans text field [in noon]
Ans text field [in evening]

Previous Next

Also on clicking 1 2 3 etc the question number will show

Perhaps you may need a plugin or script that is custom-designed for you. You may try a Google search or hire someone if you cannot do it yourself.
