You need to use the ‘wrap’ parameter at the bottom of the page, not the ‘hide’ parameter as that is for hiding one of the forms buttons.

yes I know, i’ve just been trying the different parameters to see if they work.

Sorry, by “bottom of the page” I mean that the ‘wrap’ parameter was discussed at the bottom of the page I linked to above where the shortcode parameters are detailed.

The shortcode you need to use is:

[subscribe2 wrap=’false’]

I’ve just tested it and it works for me, if it still wraps over lines for you then you need to look into your themes CSS to change the formatting.

Can you? Yes. Should you? Probably not.

The plugin is not designed to handle, process, store and use that information so it is a much bigger task than simply adding the field to the form. You need to re-write large chunks of the code to process the input and store it safely and then make use of it in the emails. Storing data which you have no intention of using is also against data protection laws in many countries.

The subscribe2 form appears to be working fine at the bottom right of the page you linked to above – did you remember to log yourself out?

yes ok but it doesn’t work on the 3rd box (where it says get your 5 tips), the code appears not the actual form.

That looks like a text widget to me, the shortcode doesn’t and won’t work from there.
