Adding new forms

I created a new form using the osticket admin interface, I can see the form name in the drop down under wp-osticket interface, but the form fields don’t show up like, they do in osticket without wordpress. My WP test site is support.permitrunner.net username: testuser pw:4y08CLvZAAE5cbIRcQEC My osticket standalone site is rylinde.com/permitrunner/request
