Continuos Host Lockout

Hi!
For some reason Ithemes Security is blocking hosts that are not attacks!
I get a beautifull white screen and the error message that I added to the Host Lockout Message.
What triggers the Host Lockout? 404 attacks? I check my apache error log and there is no attempts to open unexisting files.
How can I view what is creating these lock outs?
From admin my Ithemes security logs shows a blank page and in the FTP log folder there is nothing ( Ive chmod 777 that folder)
