Bypass Approval Process/Adding Time Stamps/Aggregate Rating Stars/User Login Req

These are some features I would like to have in my review site. Anyone been able to make these work on their site?
1. Bypass approval process – Less risk of people thinking the reviews are biased because it is being moderated
2. Time stamps – Need to let people know if something improved recently or got worse
3. Adding Stars to Aggregate Rating – I would like to better depict the average by actually showing the stars of the average customer reviews for the page instead of the text that say “Overall Rating: X out of 5 based on Y Reviews”
4. User Login Requirement – Is there a way to only allow people who are logged in to make a review to reduce bot spamming?
5. Implement time duration cookies – This is to prevent the same person from reviewing twice back-to-back (it would be cool to do a cookie that would not permit someone to post after their first post on that page until after a set time (i.e. 1 day/2 weeks/4 months)
