Page is published but cant find it in admin panel

Dear all!

Please help to solve such problem.
I have a published page in below menu (which is generated automatically) but it is not situated in pages nor somewhere else.
It shows form for sending file. It is the same in all languages althouh all rest content is being translated perfectly.

If someone helps, will send some “thank you” via PayPal
