Integrate S2Member with a Membership Form

The S2Member plugin is so versatile that I’m a bit overwhelmed with possibilities and seeking “best practices…
What I need to accomplish somehow is this:
1. A website I built and administer has a “Membership Signup” form allowing visitors to order a medication product by phone, once they are approved by staff, if they have a doctor’s letter of recommendation (for which there are form fields). Of course, this online form is automatically emailed to staff for approval. So far, so good. This has been working for months.2. Now the owner wants only visitors who’s membership form has been approved for phone sales, to also be able to buy online from a certain new “store” page of his website equipped with PayPal carts. Of course this means somehow tying access to this store page to the member’s approved membership application form.
3. Staff is willing to manually fill in some new field(s), added to the existing membership form to help allow access to the online store page.
4. So my requirement is to somehow integrate the S2Member WordPress plugin with this revised membership form.
Does that make sense?
Ideas please?
