Add Subhead Text to form with out the field

Hi rustybird, let me see if I get what you are trying to achieve. You want to add just one title at the top of the form and have all the other fields below without titles, just the entry field?

I want a subheading or title with no data entry field, just text. Following this subhead are standard Titles with data entry fields. Thank you.
