How to Enable Choosing a Gift Tag and a Message?

My products all come with a gift tag, which the customer can choose from several offered and then choose which message they want on it, or write their own.

So, here’s how the scenario works:
The customer put the item in the their cart. When they go to checkout they are prompted to choose a gift tag. ( I could in this early stage simply have a gift tag that goes with each product so they can’t choose.)

Then, much like a florist, the customer will input their message, “Merry Christmas love Fred and Amy.”

I see an extension called “Chained Product” which does the first thing, but what about the custom message? Any other suggestions?
