Plugin Not working in WP 4.0

I just discovered after upgrading to WordPress 4.0 that the “Page Links To” plugin does not work on NEWLY CREATED pages. The issue is that there is NOT the usual form field to specify the new URL.
I have tried the usual troubleshooting methods: turning off all plugins, stepping back to WP 3.9.2 etc. The problem DOES go away when 3.9.2 is reinstalled, so as far as I can determine, it is a compatibility issue with WP 4.0
Any help, insight or knowledge about resolving this problem would be greatly appreciated!
