Assessment Plugin Needed – Personalized Assessment to the User

I am looking for a plugin that would allow me to create an individual assessment. For example, after completing the questionnaire the user would receive an individualized report specific to the answers they chose.

Say I ask, ‘what medical conditions do you suffer from?’ in the questionnaire. At the end of the assessment, the user would receive feedback based on each medical condition they chose. (i.e. Since you indicated that you have Acid Reflux, we recommend…).

Essentially, I need the plugin to allow me to create a data set that links to each answer. That way, when they select a certain answer that data would display in their end report.

Are there any plugins or online programs (that I could implement into my website) that allow me to do this? I have been researching for weeks now and paid for multiple tools that ended up not giving me the functionality I need. Please help!
