    Someone else reported this issue and I wanted to say I know how to fix it.

    Issue:

    Everytime someone tries to post a comment a gray box appears. In Chrome the words “Server refused connection” can be seen.

    Cause:

    All-In-One WP Security Plugin.

    Fix:

    Disabling the plugin will not resolve the issue as it will not remove the markup in the .htaccess file.
    Instead, go to AIO WP Security > SPAM Prevention >Block Spambot Comments
    Make sure this box is unchecked.

    Thanks – it worked for me as well. All logged in users could now post.

    Unfortunately I then got the message ’duplicate comment detected it looks as though you’ve already said that’, when users without an account tried to post comments.

    I fixed that by disabling the “Add Captcha to comment forms” in WP Security > SPAM Prevention > right above “Block Spambot Comments”.
