Form Not Working & Error

  • Hi,

    We are using Newsletter Sign-Up as a shortcode in a page, in WordPress 4.0 running GridStack Child theme. Shortcode: [newsletter-sign-up-form]

    When the form is filled out and submitted, nothing happens except that it goes blank.
    It used to work.

    I tried the Config Extractor and got this:
