No user review box appearering

I’ve just installed WP Review and there is no review box appearing for customers to write reviews. The URL it’s installed on is: https://brokernotes.co/plus500-review/
As you can see, customers can only provide a star rating but can’t review specific aspects or leave a comment.
Any ideas how to solve this?
