Form Plugin wich gives out data on frontpage

Hello!

I’m looking for a plugin where I can make a form with custom fields and give out the entered data in a chart on a frontpage. Or maybe somenone knows a plugin wich stores the data in the backend and a way to post it on a site.

Does anyone know such a plugin? I’m looking for this function for weeks now. Would be really awesome!
