Send Custom Field with Form

Good morning,
I’ll use a form inside some posts. These posts have a custom field called “CODE”, that is filled by the admins when inserting the post.
When a visitor fill the Form at the bottom of post page, the FORM should be sent with the Post CODE (custom field) inside, already filled.
Can anybody help me?
The idea is send the form with a post custom field.
How can I put it automatic in my form?Thx.
