    Plugin worked fine, but then realized that it broke few OTHER forms on the site. The forms were not submitting after clicking submit. Any idea why? The form url path is in bold below.

    Sorry I have no idea. What is line 24 of page.php. Based on the error log, it seems there other form have a problem being included based on the path? I doubt that has anything to do with the plugin.

    The plugin is doing something behind the scene that affects the URL of this form, I guess. Once I disabled the plugin, the other form started working again. The other form is based on a custom page template.

    The plugin adds a rewrite rule /email/ to the end. I have no idea why the URL will break your template since when your include the template, it should not be based on URL.

    same here. When WP-Email is enabled it breaks other forms on the site. When disabled, forms work again.
