Custom post type : URL looking like /post-type/$post_name.$post_id

Hello,

I am trying to find a way to get the following param structure in my URLs (custom post type) :

/my-post-type/$post-name.$post_id
(post name then a dot -or a trailing slash eventually- then the post id).

I would like that wordpress fetch the post from it’s post’s id, and not from the post’s name (since the name is going to change often).

Any ideas ?
