    Hi Jan,

    If you set up the form fields in MailChimp (you can read how to do this in the MailChimp Knowledge Base here) and reload your list in the plugin (do this under Mailchimp > Renew Mailchimp Lists), setting up the form is actually a piece of cake.

    Under Forms, make a new form (or edit the one you already have). Under ‘Add a new field’, select the fields you want to use and insert them into your form. Save, and you’re done!

    If you want to use the form as a widget, go into the widget area and drag the Mailchimp Sign-up Form widget to the desired location. Select the form you’ve made and click on save. It’s that easy!
