Closest location to user no longer working

We’ve setup a bunch of locations using a custom post type. I previously had it working using one of the GMW forms where a user could ‘get their location’ and then the closest location would popup in the site header. I don’t know when, but this has stopped working. I’ve tried playing around with a variety of the shortcodes, and all the settings in the Forms, but with no luck. Any thoughts?
