Fatal error when trying to activate

Just as the title says, whenever I try to activate this plugin on my site (using Multisite, in case that makes a difference), it throws a fatal error in the Plugin page. Not only that, but trying to delete the plugin from the same console leads me to a “Are you sure you want to do this?” section. It’s very bugged.
Originally I wanted to try it and see if I could implement it on the Buddypress register page, but I guess I’ll have to wait to see if this gets fixed first.
