No fields are showing up

Hi there

I am running the Foodie Pro Theme through Genesis.

I tried to use this plugin to connect it with my mailchimp account and have followed the tutorials perfectly but the fields just don’t seem to show up on my home page. As instructed, I copied the form action part of the code into the plugin settings and typed in EMAIL under email. I’ve been told this may be caused by a caching issue but I have no idea what that means. Please could you give me some suggestions to try out as I would like to start capturing emails on my site. Thanks!
