Fail Response and attachments

First:

“Failed to send your message. Please try later or contact the administrator by another method.”

I got it to work once. But the email I got had the information layed out without titles. So I went in and simply added things like “Phone:” in front of the ‘[phone]’ code.

After doing this, I went back and tested the field again but I get the failed response I quoted above. What do I do?

Second:
I have three attachment buttons on my form. When the form worked, it simply put the title of the attachment in the email but did not include the attachment itself.
How do I get the actual image to display?
