Form Quantity Calculator

Hello

I’m wondering if there is a simple plugin which allow the following.

Create form with text area and quantity fields that get the quantity entered and automatically update the total. The submit button would just sent all fields to me by email

[Product 1] [Price Per Unit] [Enter Quantity]
[Product 2] [Price Per Unit] [Enter Quantity]
[Total] [Calculated Total]
[Submit]

Further example
