How can I pick up the input from a field in the form to reply?

I really like this plugin but I have a scenario that ideally requires the me to pick up a textbox’s input and put it in the auto reply email subject/body.
Here is my site’s registration linke:
I want the auto reply email including some specific fields on the form, for example, the name field, and put the field’s value in the auto email’s subject and/or body.
Can anyone tell me how to do that? Which file is generating the auto email?
