I need a subscription form like this

Hello! I have a site up at goodschooling.net. At the top and bottom there is a very simple email subscription form. Just an inline email and submit button. I did this using Beaver Builder, which has an “element” for it. It’s perfect. But I am building another site with Divi where I want something as close to this as possible Divi’s form is not helpful, only integrating with three mail services, none of which I use. I am using GetResponse.

Anyways, can anyone tell me if they know of a way to replicate this form? I thought there must be plugin in out there, but I just can’t find anything. I am new to all this and may not be hitting the right keywords, but it’s driving me crazy. Hopefully someone with more experience can give me some direction.
