Yes, I have contact-form-7.4.2.1. You can get to it by clicking on “contact” in the sidebar menu. I don’t recognize the contact form that pops up on top.

Hi again. Never heard back from anyone on this. Would love some help. My site: I’ve updated to Twenty Twelve version 1.9. I removed my custom sidebar menu, but I’m having the same problem with the default menu bar at the top of the theme. Every tab causes a contact form to pop up. What gives?

That was the clue I needed! I didn’t realize I had two contact form plug-ins installed. I deactivated SimpleModal Contact Form and now the menu works! (Now, I just have Contact Form 7 activated.) Thanks!
