App/Tab doesn't show up on facebook tab manager page

Greetings,
I followed the setup instructions as best I could considering that fb has changed its interface since instructions were written. Need help with forms to get facebook to recognize app.

1. My site has an SSL but I didn’t see where to enter it.

2. What do I put in the field for Facebook Profile ID? My page has a custom URL “ikidyunot”. Please tell me exactly what I should put in that field.

3. How do I fill in the field for Facebook Canvas Page URL? Instructions says NOT INCLUDING “https://apps.facebook.com/”

4. When I click on the Visit this app profile link next to Facebook App ID field, I get a Page Not Found page. I know that’s a clue but I don’t know what it tells me.

5. I didn’t see the fields listed in the WPB instructions, in the facebook setup process. What do I need to do on facebook to get facebook to recognize the app? I created the App ID and App Secret code.

6. App does not show up on Tab Manager page. I logged into developers.fb.com and made the app available to the public on the Status and Review tab. What else do I need to do on facebook?

Please help; I’d really like to get this working.
