Login Error Msg BEFORE attempting to login

  • On one of the sites we maintain, the message:

    ERROR: Invalid username or incorrect password.

    is appearing at the top of the login page before the user even attempts to login. One can login ok in spite of the message, but the error message is confusing users. Any ideas? Here’s the page:

    is there a plugin that is in use to customize the login page? might be connected to the error msg you are seeing.
    (i see a custom logo showing)
