Horrendously slow on cloud hosting?

I’m having massive speed issues with a woocommerce site on cloud hosting.
It’s gotten progressively slower over time, and has now hit a brick wall, time to first byte is around 13 secs. If I disable all plugins and switch to default theme, the load time remains bad. Other sites on the same server run fast. wp_options is around 180,000 rows and wp_postmeta is in the same ballpark. I think wp_options is the culprit.
If I back up the db and run it as an identical mirror locally (MAMP) it’s fine, even though wp_options remains in the same ballpark of number of rows.
My thought is that this may be due to some issue (related to the database) with saving and deleting the transients woocommerce uses (ie. not happening properly).
I’m curious if anyone else has experienced this or has any advice (?) as it’s losing my client a ton of sales.
