Can't get rid of Version in URL

I’m having trouble getting rid of ?v=XXXXX from my URL. I’ve tried a few plug-ins that are supposed to hide the version tags, but none have worked. The problem has started with WordPress 4.3. Any idea where this is hard-coded and can be removed?
