This happens from time to time and I’m not exactly sure why. The resolution is to re-write your permalinks. To do so, do this:

    1. Go to your Permalinks setting and save your current setting (copy the value for safe keeping).
    2. Set the permalinks back to the default setting and save your settings.
    3. Set your permalinks back to their desired setting with the value you saved.
    4. Test your form URL.

This should resolve the problem as it forces a permalinks re-write which usually clears this up.

Thanks figured it out. Also when I set it up for multi columns it does not change that when it is viewed on mobile. Is there a way to make it responsive?

    thank you for your help

I assume what you are asking is for the form to appear in multiple columns when the browser is wide enough but automatically collapse when it isn’t.

No, there isn’t any way to do this automatically that I know of. If you are really cleaver with CSS you might be able to undo what the jQuery Columnizr plugin does.

The right way to do it would be for the plugin to detect when the screen isn’t wide enough and ignore the column request. If there is a simple way to do that with jQuery I could look into it but I am not aware of anything off the top of my head.
