Need Plugin sugggestion

    I am not sure of what plugin I need… But here is what I wish to achieve. I am putting together a healthcare website which is more like an App (make quick selection to display an output). Here are the steps that visitor have to go through:

    1) Three are three symptoms and my visitors are required to identify one.
    2) There are six therapy areas per symptom and my visitors are required to opt for one.
    3) There are more questions within each therapy area that my visitor has to answer all.
    4) Based on all inputs my site will display a remedy.

    Is there any recommended template/ plugin for my website?
